Hospice awarded community health accreditation

Community Hospice and Health Services, the oldest nonprofit hospice and support services organization serving the Central Valley, recently announced hospice reaccreditation and palliative care recertification by Community Health Accreditation Partner (CHAP).

CHAP is the leading national accrediting agency for hospice and home health organizations.

This recognition reaffirms Community Hospice & Health Services’ ongoing commitment to providing the highest standard of compassionate, patient-centered care to individuals and families throughout the region.

CHAP accreditation and certification represent a rigorous, objective evaluation of an organization’s operations, ensuring compliance with nationally recognized standards of excellence. The purpose of CHAP is to confirm the excellence of community healthcare practice through consistent measurement of the delivery of quality services. CHAP assesses all areas of an organization including organizational leadership, clinical quality, patient outcomes, resources, compliance, community impact and long-term viability. Every three years Community Hospice and Health Services is surveyed by CHAP for continued accreditation and certification.

Community Hospice and Health Services (CHHS) is a nonprofit, community-based agency, serving the greater Central Valley since 1979. CHHS embraces more than 2,500 patients and families each year providing support through vital programs including hospice and palliative care for adults and pediatrics, general inpatient care and respite care at the Alexander Cohen Hospice House, hospice family bereavement support, Hope Counseling outpatient mental health counseling for adults and children, and Camp Wokini, a youth camp for children and teens who have experienced loss.

CHAP is an independent, nonprofit organization accrediting providers of home and community-based care. Founded in 1965, CHAP was first to recognize the need for and value of home and community-based care standards and accreditation. As a Centers for Medicare and Medicaid Services (CMS)–approved accrediting organization, CHAP surveys organizations providing home health, hospice, and home medical equipment services to establish if Medicare Conditions of Participation and DMEPOS Quality Standard are met and recommend certification to CMS.
